Medical Insurance
CIS pays 100% of the employee's medical insurance premium and a portion towards dependent coverage, which is a pre-tax deduction. The employee has the opportunity to elect coverage that meets individual or family needs. Our medical plan is a Preferred Provider Plan and is subject to location and availability. Under this flexible plan, employees are not required to select a primary care physician and may go to a specialist without having a referral.
Vision Insurance
CIS includes vision plan coverage as part of the medical plan for employees and dependents. The plan allows employees to select the doctor of your choice. The plan includes an exam, lenses, and frames.
Dental Insurance
CIS pays 100% of the employee's dental insurance premium and a portion towards dependent coverage, which is a pre-tax deduction. The employee has the opportunity to elect coverage that meets individual or family needs.
The dental insurance plan allows the employee to select their dentist of choice. The following services are covered at the listed percentages:

The maximum benefit is $1,500 per calendar year for each covered person. The calendar year deductible is $50 per person with a maximum of $150 per family, and the deductible is only applied to restorative care and major care.
